The forms below are used to participate in both group build days and individual build days.

		Volunteer Manual - This document includes all the details on volunteering including all the forms you need to complete.
		Blue Hill Place Group Registration Form - Please complete and submit this form to the Habitat Greater Boston office to register for a group build date at the Blue Hill Place site. Habitat Greater Boston cannot confirm your build date without this completed form.
		Newton Group Registration Form - Please complete and submit this form to the Habitat Greater Boston office to register for a group build date at the Newton site. Habitat Greater Boston cannot confirm your build date without this completed form.
		Safety Checklist - We ask that all volunteers read this checklist so that they can better understand Habitat Greater Boston’s approach to safety on site.
		Individual Volunteer Waiver of Liability Form - Please bring this completed form to the build site on your build day. Volunteers 16 or 17 years old must include a parental signature.
		Group Waiver of Liability Form - Please bring this completed form to the build site on your build day. It must include signatures from all participants. Volunteers 16 or 17 years old must include a parental signature.
		Pledge Sheet - You may wish to use this sheet to gather pledges of support from co-workers, friends, and family.
		Orientation Registration Form - Please complete this online form to attend one of the mandatory volunteer orientation sessions.
